Current Name of Applicant: Enter the applicant's current name, including first, middle, last name, and suffix if applicable.
Approximate Date Current Name Was Assumed: Provide the month and year when the current name was adopted.
Former Name of Applicant: List the applicant's previous name, including first, middle, last name, and suffix if applicable.
Applicant's Date of Birth: Enter the applicant’s date of birth in the format mm-dd-yyyy.
Applicant's Place of Birth: Specify the city and state if born in the U.S., or the city and country if born outside the U.S.
Number of Years You Have Known the Applicant: State how long you have known the applicant.
Your Relationship to the Applicant: Indicate your relationship to the applicant (e.g., friend, relative).
Known by Current Name and Former Name: Confirm that you have known the applicant by both names.
Explanation of Name Variance: Explain the difference between the applicant’s birth name and current name, and state if the current name is used exclusively.
Affiant’s Declaration: Swear (or affirm) that the information provided is true to the best of your knowledge. State that the applicant is known by both names and is the same person.
Printed Name of Affiant: Print your name legibly.
Affiant’s Signature: Sign the form in the designated area.
Affiant’s Social Security Number: Provide your Social Security number.
Address of Affiant: Enter your address, including street, city, state, and zip code.
Identifying Document Submitted: Specify the type of identification you are submitting (e.g., driver's license), including the date of issuance and expiration, and document number.
Notary Section: The notary public or passport agent must complete this section, including the date and location of notarization, their name, and seal.
Additional Comments: Use the back of the form if additional space is needed for explanations.
