Employee Information:

  • Enter the employee's name, address, and any relevant identification details such as employee ID, business unit, and pay group.

Pay Period Details:

  • Specify the issue date, check number, pay period begin and end dates, and any additional information regarding taxes, filing status, allowances, and YTD (Year-to-Date) hours and pay.

Pay Description:

  • Describe the pay breakdown for different categories such as regular pay, overtime, holiday pay, personal days, vacation, and any additional bonuses.

Tax and Deduction Details:

  • Include information on federal and state tax withholding, as well as any other deductions such as for sick leave, FMLA, and bonuses. Specify both current taxable pay and YTD taxable pay, along with current taxes withheld and YTD taxes paid.

Benefits and Deductions:

  • List any before-tax deductions for benefits like medical, dental, vision, life insurance, and after-tax deductions. Include employer-paid benefits and their corresponding amounts.

Gross and Net Pay:

  • Calculate the total gross pay before deductions, total taxes, deductions, and finally, the total net pay. Ensure accuracy in all calculations.

Distribution of Net Pay:

  • Break down the distribution of net pay, including any allocations for vision benefits, short-term disability, long-term disability, and other benefits or contributions.
